Together, we look at what’s already working, where you need more support and what you’d like to change. You have someone to discuss options with, ask questions of and work through the practical steps alongside you.

Here’s a closer look at who we work with, how we get started and what you’ll need to access our support.

Support Coordination is for NDIS participants who have funding for this service in their plan and would like help understanding, organising and managing their supports.

 

You might be putting a new plan into practice, exploring different providers or finding that your current arrangements need a rethink. Perhaps there are gaps in your support, several services to coordinate or changes in your life that need to be considered. We start with what’s happening for you and where some practical guidance could make a difference.

 

You can also work with us to build your confidence in managing supports over time. That might mean becoming more comfortable asking providers questions, understanding your agreements or speaking up when something isn’t working.

 

We work with participants across Hunter New England and Regional NSW. Families, carers and other trusted people are welcome to be involved in the ways you choose, with your preferences guiding what information is shared and how we work together.

We begin by getting to know you and talking about your plan, current supports and priorities. We want to hear what’s going well, what feels difficult and what you’d like to happen next. From there, we agree on what you’d like help with and the steps we’ll take together.

 

This may include exploring providers, helping you understand service agreements or coordinating communication between the people involved in your support. We can help you consider different options and prepare questions, so you have useful information when making decisions.

 

You stay involved throughout. We explain what we’re working on, discuss the options with you and seek your permission before involving others or sharing your information.

 

As things progress, we look at how your arrangements are working in everyday life. If your needs change or a service isn’t the right fit, we help you work through the issue and consider adjustments. Along the way, we support you to build skills and confidence in directing your own support.

You can contact us directly or ask someone you trust to make a referral with your permission. This could be a family member, carer or professional already involved in your support.

 

Have your NDIS plan available if you can, so we can discuss the relevant funding and what you’re looking for. It’s also helpful to tell us about your current providers, any changes you’re hoping to make and anything that needs attention soon. If you’re unsure what information to provide, we can talk it through with you.

 

We’ll discuss whether our service is a suitable fit, answer your questions and explain the next steps before you decide to proceed. This is also your opportunity to get to know our approach and tell us how you prefer to communicate or who you’d like involved.
